The Director of Care Transitions reports to the Chief Nursing Officer and is responsible for leading our client's care transitions strategy. This includes overseeing case management, social work, and discharge planning functions in inpatient settings. The Director manages a team of approximately 60 FTEs and ensures that care coordination activities align with organizational priorities such as quality, patient experience, operational performance, regulatory compliance, and financial stewardship.In this role, the Director is tasked with designing, implementing, and continuously improving systems and processes that support safe, timely, and effective transitions of care. The goal is to ensure that patients receive the right care at the right time and in the most appropriate setting, while reducing unnecessary variation, avoidable delays, readmissions, and barriers to discharge. The Director plays a key role in optimizing patient outcomes and enhancing the overall efficiency of care delivery within the organization.
