Description

The client is establishing an Academic Family Health Team (AFHT), a new primary care clinic serving Western York Region, using an interprofessional model of care. The AFHT is expected to improve access to comprehensive, team-based primary care, support patient attachment, and advance education, research, and innovation in partnership with health system and academic collaborators. The model will bring together family physicians and allied health professionals to deliver coordinated care responsive to patients’ health and social needs, supported by an integrated electronic medical record and shared digital tools to strengthen collaboration, continuity of care, and the experience of patients and caregivers.

The client is seeking four inaugural Board Directors to help establish the organization’s governance foundation and shape its future direction. Board Directors will work with fellow Directors and organizational leadership to provide strategic oversight, steward the organization’s mission and resources, and support the delivery of accessible, high-quality, patient-centred primary care. Directors will help define the Board’s culture, practices, and priorities while ensuring effective governance, accountability, and responsible decision-making, including fiduciary responsibilities related to strategic planning, financial and risk oversight, quality and performance monitoring, and governance policy development.

Board Directors are expected to bring thoughtful judgment, an understanding of the communities served, and a commitment to collaboration, equity, and continuous improvement. As ambassadors for the organization, Directors will help strengthen relationships and advance the AFHT’s role within the broader health system, academic community, and local community. The client seeks experienced candidates with demonstrated board experience and a broad spectrum of governance competencies, including strategic planning, financial oversight, risk management, quality and performance, human resources, health system knowledge, community engagement, and organizational development, with particular priority for at least one Director who is a Chartered Professional Accountant (CPA) and at least one Director who is a Lawyer.
