The Chief and Medical Director of Health Equity and Population Health is responsible for leading the integration of population health and health equity initiatives within our client's system. This role involves developing a vision and strategy for advancing population health in alignment with the organization's values and mission. Reporting to the Chief of Staff and Health System Executive - Clinical, the Chief and Medical Director will collaborate with various internal and external stakeholders to promote quality and health equity principles across all clinical programs and medical departments.The Chief and Medical Director will research best practices in population health and health outcomes, and provide guidance on implementing strategies to improve patient outcomes and address health disparities within Durham Region. This includes developing and monitoring a measurement strategy to track progress in addressing population health outcomes and health equity improvement efforts. Overall, the Chief and Medical Director will play a crucial role in advancing our client's services, culture, and practices to better support the health and well-being of communities in the region.
